When people ask me how I invented the Pinhole® Surgical Technique, I always feel that the story should begin with the people whose work came before mine.

No surgical technique develops in isolation. We build upon the discoveries of those who preceded us. In my case, I was fortunate to learn from some of the giants of periodontal surgery and microsurgery.

Standing on the Shoulders of Giants

An important milestone in the treatment of gum recession came from Drs. Burton and Laureen Langer. In 1985, they published their landmark description of the subepithelial connective-tissue graft for root coverage.

Their work helped establish connective-tissue grafting as an important and highly successful approach to treating gum recession. Many periodontal surgeons subsequently refined these concepts and developed sophisticated techniques capable of producing excellent results.

Another major influence on my thinking was the late Dr. Dennis Shanelec, one of the pioneers of periodontal microsurgery.

Dr. Shanelec introduced me to the surgical microscope and helped me appreciate an idea that eventually became central to my own work: the less unnecessary trauma we create during surgery, the better the experience may be for the patient.

Microsurgery demonstrated that smaller instruments, greater precision, and more delicate tissue handling could accomplish surgical objectives while reducing tissue trauma.

That principle stayed with me.

A Lesson From Another Area of Dentistry

There was another experience that influenced the development of Pinhole.

In performing endodontic procedures, I was familiar with the small access approach used in certain root-end surgical procedures. I became impressed by how much could sometimes be accomplished through a relatively limited surgical opening.

That led me to a simple question: Could gum recession also be treated through a very small access point rather than through conventional surgical incisions?

I was not trying to replace periodontal plastic surgery. I was exploring whether there might be another way to accomplish the same treatment objective while making the procedure less invasive for patients who might prefer that approach in their particular situation.

The Idea of a Pinhole

The concept gradually became clearer.

Instead of cutting and reflecting the gum tissue in the conventional manner, what if I could enter through a tiny opening and carefully release the tissue underneath?

And if the tissue could be released sufficiently, could it then be gently advanced over the exposed root?

The idea sounded simple. Making it work was not.

Conventional dental instruments were not designed to perform this procedure through such a small opening. I therefore began developing specialized instruments that would allow me to work underneath the gum through a tiny access point.

Through repeated development and refinement, the concept that eventually became the Chao Pinhole® Surgical Technique began to take shape.

The objective was straightforward: Use the smallest practical access, minimize surgical trauma, preserve the existing gum tissue, and reposition that tissue to cover the exposed root—without conventional cutting or suturing.

An Additional Choice for Patients

Connective-tissue grafting and other periodontal plastic-surgical procedures remain important treatments for gum recession and have decades of scientific and clinical support.

Pinhole was developed as an additional minimally invasive option, giving patients and doctors another way to approach the treatment of gum recession.

Each patient and each recession condition presents its own unique characteristics. The choice of treatment is best made after careful evaluation and discussion between the patient and the treating doctor, considering the available options and the patient's preferences for that specific situation.

The Principle That Still Guides Me

Looking back, I do not consider Pinhole an idea that appeared suddenly. It was part of an evolution.

The Langers demonstrated what could be accomplished with connective tissue and root-coverage surgery. Periodontal surgeons continued to refine those procedures. Dennis Shanelec and other pioneers of periodontal microsurgery demonstrated the value of precision and minimizing surgical trauma.

I was fortunate enough to learn from that history.

My contribution was to ask one additional question: How much of the traditional surgical access could we eliminate and still accomplish the goal?

That question ultimately led to the Pinhole® Surgical Technique.

More than two decades later, I am still guided by essentially the same principle: Preserve as much tissue as possible. Create as little surgical trauma as necessary. And never stop looking for a simpler and gentler way to accomplish the treatment objective.
